新一代A酸 Altreno、 Arazlo 到底夯什麼？聽說降低刺激性，九歲以上孩童也可以使用？

Altreno : tretinoin lotion, 0.05%

Tretinoin可調節表皮細胞的生長及分化。擦在皮膚可減少毛囊開口過度角化，也就是降低粉刺的生成 ; 也可增加毛囊表皮細胞的代謝，促進粉刺的排出。
新一代第一代外用A酸，一天使用一次，比較特別是早上也可以擦，擦完再化妝也沒關係，但切記防曬一定要做好。這款Ａ酸跟傳統tretinoin乳霜或凝膠比起來，添加了保濕的成分：玻尿酸、甘油和水溶性膠原蛋白，可減少乾燥脫皮的現象。特別要注意的是，裡面還有從魚類萃取的蛋白，如果對魚有過敏的患者，應謹慎使用！
療效方面跟對照組相比，不管是發炎性的痘痘，或非發炎性的病灶像是粉刺，皆有顯著改善的效果。在第12週，發炎性的病灶可減少50%以上，非發炎性的病灶減少45%以上。至於在第12週的治療成功率，兩篇研究各顯示了16.5%跟19.8%！

Arazlo : tazarotene lotion, 0.045%

Tazarotene擦在皮膚上可快速轉化成有效成分(tazarotenic acid)，降低身體的吸收率，並且讓皮膚角質生成正常化、減少發炎、促進膠原蛋白增生、調理疤痕生成跟發炎後色素沈澱。
新一代第三代外用A酸的劑型有特殊的蜂巢結構，可以讓藥劑更均勻地分佈在皮膚表面，同時裡面的保濕成分可在皮膚形成保護層，避免水分散失。比較重要的是這藥物有致畸胎性，懷孕不能使用，甚至用藥前兩週就要驗孕確定沒有懷孕才可以使用。再者，病人若同時有在使用其他口服藥物，要先跟皮膚科醫師說明，才能避免跟一些容易增加光敏感性的藥物合併使用(thiazides, tetracyclines, fluoroquinolones, phenothiazines, sulfonamides)
療效方面跟對照組相比，在第12週發炎與非發炎的病灶都可減少60%以上，而且將近三成的患者治療成功！跟傳統第三代tazarotene cream 0.1%比起來，雖然濃度不到一半，但是成效是相當的喔！此外，病人的耐受度更高、副作用的比例更少。就算出現皮膚不適，最多就是在使用後的兩個星期內，之後發生率就逐漸下降。
